LICENSED PRACTICAL NURSE

Signature HealthCARE is seeking a compassionate Licensed Practical Nurse to join our long-term care and post-acute team. In this role, you'll deliver hands-on, person-centered nursing care, administer medications and treatments, monitor resident conditions, and support rehabilitation goals. Responsibilities

• Provide direct nursing care to long-term care and post-acute residents under RN supervision
• Administer medications and treatments per physician orders and facility protocols
• Monitor, document, and report changes in resident condition using EHR systems
• Support rehabilitation and restorative care plans to promote resident independence
• Collaborate with interdisciplinary team to develop and update person-centered care plans
• Educate residents and families on care, treatments, and health management
• Ensure compliance with infection control, safety, and regulatory standards
• Assist with admissions, discharges, and resident assessments
• Promote resident dignity, comfort, and engagement in a faith-based, mission-driven environment
• Participate in ongoing training, quality improvement, and team huddles
