LICENSED PRACTICAL NURSE

Spring City Care and Rehab Center is seeking a Licensed Practical Nurse to provide compassionate, resident-first care in our skilled nursing and rehabilitation facility. The LPN will deliver hands-on clinical care, including medication administration, treatments, and monitoring, while supporting personalized care plans for short-term rehab, long-term care, and memory care residents. Responsibilities

• Provide compassionate bedside nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments in accordance with physician orders and LPN scope of practice
• Monitor, document, and report changes in resident condition using EHR systems
• Support development and implementation of individualized, resident-centered care plans
• Collaborate closely with RNs, therapists, and support staff to deliver coordinated, interdisciplinary care
• Assist with admissions, discharges, and resident and family education
• Promote infection control, safety, and quality standards throughout the facility
• Support residents' dignity, independence, and emotional well-being in a homelike environment
