LICENSED PRACTICAL NURSE

Signature HealthCARE seeks a compassionate Licensed Practical Nurse to deliver resident-centered care in our long-term and post-acute facilities. In this role, you'll provide direct nursing care, administer medications and treatments, monitor resident conditions, and accurately document in the EHR. You'll collaborate with an interdisciplinary team to support individualized care plans, rehabilitation goals, and quality outcomes. Responsibilities

• Provide direct resident care including assessments, treatments, and medication administration
• Monitor, document, and report changes in resident condition in the EHR system
• Collaborate with RNs, physicians, therapists, and interdisciplinary team to implement care plans
• Support rehabilitation and post-acute care goals through timely, accurate clinical interventions
• Educate residents and families on care plans, medications, and health management
• Uphold infection control, safety, and regulatory compliance standards
• Participate in rounds, care conferences, and quality improvement initiatives
