LICENSED PRACTICAL NURSE

Signature HealthCARE seeks a compassionate Licensed Practical Nurse to provide resident-centered care in our long-term and post-acute communities. In this on-site role, you'll administer medications and treatments, monitor clinical status, document in EHR, and support individualized care plans as part of an interdisciplinary team. You'll help residents achieve their highest level of function while modeling dignity, compassion, and servant leadership in a faith-based, mission-driven culture that supports ongoing training, collaboration, and career growth.

Responsibilities

• Provide safe, person-centered nursing care to long-term and post-acute residents
• Administer medications and treatments per provider orders and LPN scope
• Monitor, document, and report changes in resident condition using EHR systems
• Support care planning with the interdisciplinary team to meet resident goals
• Educate residents and families on care plans, treatments, and health needs
• Follow infection prevention, safety, and quality standards at all times
• Collaborate with RNs, therapists, and caregivers to promote optimal outcomes
• Model compassion, dignity, and servant leadership in daily interactions
