LICENSED PRACTICAL NURSE

Signature HealthCARE seeks a compassionate Licensed Practical Nurse to provide resident-centered care in our long-term care and post-acute rehabilitation community. In this on-site role, you will administer medications and treatments, monitor and document resident status, and support individualized care plans in collaboration with an interdisciplinary team. Responsibilities

• Provide direct LPN nursing care to residents in long-term care and post-acute settings
• Administer medications and treatments per physician orders and facility protocols
• Monitor, document, and report changes in resident condition using EMR systems
• Support interdisciplinary care plans focused on person-centered, faith-informed care
• Educate residents and families on care plans, treatments, and health management
• Maintain infection control, safety, and quality standards in all clinical activities
• Collaborate with RNs, therapists, and care staff to promote optimal resident outcomes
• Participate in ongoing training, quality improvement, and team meetings
