Responsibilities
- Perform comprehensive patient histories and physical examinations for diverse populations.
- Assess, diagnose, and manage acute and chronic conditions using evidence-based guidelines.
- Develop, implement, and adjust individualized treatment plans, including prescribing medications within scope of practice.
- Order, review, and interpret diagnostic tests and laboratory results, communicating findings clearly to patients and the care team.
- Provide patient and family education on diagnoses, medications, treatment plans, and preventive care strategies.
- Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate care across settings.
- Document all clinical encounters accurately and timely in the Electronic Health Record (EHR) in accordance with regulatory and organizational standards.
- Participate in quality improvement initiatives, case reviews, and team meetings aimed at enhancing patient outcomes and clinic efficiency.
- Support population health efforts by addressing care gaps, facilitating follow-up, and promoting adherence to treatment plans.
- Maintain current knowledge of clinical best practices, regulatory requirements, and professional standards relevant to advanced practice nursing.
