Licensed Vocational Nurse

Touchstone Communities is seeking a compassionate Licensed Vocational Nurse to join our skilled nursing and rehabilitation team in Texas. In this on-site role, you'll deliver person-centered clinical care, administer medications and treatments, monitor residents' conditions, and document in EMR systems. You'll collaborate closely with RNs, therapists, CNAs, and physicians to support recovery, long-term care, and quality of life in a homelike, faith-based environment. LVNs at Touchstone grow clinical and leadership skills while serving residents and families with dignity, respect, and servant leadership.

Responsibilities

• Provide direct nursing care to residents under RN and physician direction
• Administer medications and treatments per providers' orders
• Monitor and document vital signs, symptoms, and resident status in EMRSupport care planning and interdisciplinary care conferences
• Perform wound care and basic rehabilitative nursing interventions
• Educate residents and families on care plans and health needs
• Promote infection control and resident safety standards
• Collaborate with CNAs, therapists, and providers to optimize outcomes
• Respond to changes in condition and escalate concerns promptly
• Support a faith-based, resident-centered culture of dignity and respect
