Nurse Practitioner/Physician Assistant

CenterWell Home Health, part of Humana Inc., seeks a PRN Nurse Practitioner/Physician Assistant to deliver high-quality in-home care to seniors and adults with chronic conditions. In this role, you will perform comprehensive assessments, manage acute and chronic illnesses, and develop individualized care plans that help patients recover, manage symptoms, and remain independent at home. You'll collaborate closely with an interdisciplinary team, use advanced clinical tools, educate patients and caregivers, and contribute to a compassionate, patient-first culture focused on safety, respect, and excellent outcomes.

Responsibilities

• Provide in-home assessments, diagnoses, and treatment plans for adult and senior patients
• Perform physical exams, order and interpret diagnostic tests, and adjust care plans
• Collaborate with an interdisciplinary team to coordinate personalized home health care
• Educate patients and caregivers on condition management, medications, and safety in the home
• Document visits, care plans, and outcomes accurately in electronic health records
• Communicate changes in patient status to physicians and care team promptly
• Ensure compliance with clinical protocols, quality standards, and regulatory requirements
• Participate in care conferences and contribute to continuous improvement of patient outcomes
