Physical Therapist / PT, Home Health

Responsibilities

• Conduct comprehensive physical therapy evaluations in patients' homes
• Develop and implement individualized treatment plans and goals
• Provide therapeutic exercises, gait and balance training, and functional mobility interventions
• Educate patients and caregivers on home exercise programs and safety strategies
• Collaborate with physicians, nurses, and other clinicians to coordinate care
• Accurately document assessments, treatments, and progress in EMR systems
• Monitor, reassess, and adjust interventions based on patient response
• Ensure compliance with Medicare, payer, and home health regulatory requirements
• Participate in case conferences and quality improvement initiatives
• Maintain professional development and uphold clinical best practices
