Licensed Practical Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Licensed Practical Nurse to provide high quality nursing care for short term rehab and long term care residents. The LPN will deliver direct patient care, administer medications and treatments, monitor vital signs, document accurately, and support individualized care plans under RN and physician direction. This role collaborates closely with an interdisciplinary team, communicates with families, and helps maintain a safe, homelike environment. Ideal candidates are resident centered, detail oriented, and committed to dignity, respect, and continuous clinical improvement.

Responsibilities

• Provide direct nursing care to rehab and long-term care residents
• Administer medications and treatments per provider orders
• Monitor and document vital signs, conditions, and responses to care
• Assist with assessments and implementation of individualized care plans
• Collaborate with RNs, therapists, and physicians on resident care
• Communicate effectively with residents and their families
• Maintain accurate, timely electronic and paper documentation
• Support ADLs and ensure resident comfort, safety, and dignity
• Follow infection control, clinical, and safety protocols
• Contribute to a resident-centered, team-based care culture
