Nurse Practitioner

Senior Community Care is seeking a compassionate Nurse Practitioner to provide primary and preventive care to seniors enrolled in our PACE program. In this interdisciplinary role, you will perform assessments, diagnose and manage acute and chronic conditions, prescribe medications, and develop individualized care plans. You'll collaborate closely with physicians, nurses, social workers, therapists, and caregivers to support safe, independent living. Responsibilities

• Provide primary and preventive care to PACE-enrolled seniors
• Conduct comprehensive health assessments and routine follow-up visits
• Diagnose and manage acute and chronic medical conditions
• Prescribe medications and order/interpret diagnostic tests
• Develop, implement, and adjust individualized care plans
• Collaborate with an interdisciplinary care team to coordinate services
• Educate participants and caregivers on disease management and wellness
• Document care in the electronic health record accurately and timely
• Participate in care conferences and quality-improvement initiatives
• Support safe, independent living and transitions of care for seniors
