Physical Therapist

FOX Rehabilitation is seeking a Physical Therapist to provide evidence-based, functional care to older adults in their homes and senior living communities. In this full-time role, you will perform comprehensive evaluations, develop individualized treatment plans, deliver one-on-one interventions, and document outcomes using EMR. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and prioritize safety, mobility, and independence. Responsibilities

• Provide one-on-one physical therapy to older adults in homes and senior living communities
• Perform comprehensive evaluations and functional mobility assessments
• Develop and implement individualized, evidence-based treatment plans
• Document care, progress notes, and outcomes in EMR systems
• Educate patients, families, and caregivers on home exercise and safety
• Collaborate with an interdisciplinary team to coordinate patient care
• Focus on fall prevention, balance training, strength, and functional independence
• Participate in ongoing clinical education, mentorship, and quality initiatives
