Physical Therapist

FOX Rehabilitation seeks a full-time Physical Therapist to provide evidence-based care to older adults in homes and senior living communities in Sioux City, IA. Evaluate functional mobility, design and deliver individualized treatment plans, and focus on fall prevention, strength, balance, and safety. Responsibilities

• Evaluate older adults' functional mobility, balance, strength, and safety in home and senior living settings
• Develop and implement individualized, evidence-based physical therapy treatment plans
• Focus on fall prevention, functional mobility, and improved independence in daily activities
• Educate patients, families, and caregivers on exercises, home safety, and long-term wellness
• Document assessments, goals, progress, and outcomes in the EMR accurately and timely
• Collaborate with an interdisciplinary care team to coordinate comprehensive geriatric care
• Utilize productivity tools and clinical resources to optimize care delivery and scheduling
• Participate in ongoing education, mentorship, and quality improvement initiatives
