FOX Rehabilitation is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based geriatric rehabilitation, focusing on improving patient mobility, function, and overall quality of life across a variety of care settings. You'll collaborate within an interdisciplinary team to develop and implement individualized treatment plans, document progress, and educate patients and caregivers. Responsibilities

  • Perform comprehensive evaluations of geriatric patients, including functional mobility, strength, balance, and safety assessments
  • Develop and implement individualized, evidence-based treatment plans focused on improving function, independence, and quality of life
  • Provide therapeutic interventions such as therapeutic exercise, gait and balance training, functional mobility training, and pain management strategies
  • Educate patients, families, and caregivers on home exercise programs, safety, fall prevention, and strategies to maximize independence
  • Collaborate with an interdisciplinary team, including physicians and other healthcare professionals, to coordinate and optimize patient care
  • Document all evaluations, treatments, progress notes, and discharge summaries accurately and promptly in the electronic medical record
  • Monitor, measure, and document patient outcomes using objective metrics to guide clinical decision-making and adjust plans of care
  • Manage a field-based caseload, organize daily schedule, and travel between patient homes or community settings as required
  • Adhere to all professional, ethical, regulatory, and organizational standards of practice for physical therapy
  • Participate in ongoing professional development, mentorship, and training opportunities to maintain and enhance clinical skills
