Physical Therapist

Responsibilities

• Evaluate geriatric patients and develop individualized, evidence-based treatment plans.
• Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Document assessments, goals, progress notes, and outcomes in compliance with payer and regulatory standards.
• Collaborate with an interdisciplinary team to coordinate patient-centered care.
• Educate patients, families, and caregivers on home programs, safety, and fall prevention.
• Participate in professional development and clinical education activities.
• Contribute to a culture of respect, clinical excellence, and continuous improvement.
