Physical Therapist

FOX Rehabilitation is seeking a compassionate Physical Therapist to provide evidence-based geriatric rehabilitation services. In this clinician-owned practice, you will evaluate and treat older adults in a variety of settings, develop individualized treatment plans, and focus on functional outcomes that enhance quality of life. Collaborate with an interdisciplinary team, document care accurately, and educate patients, families, and caregivers. Responsibilities

• Evaluate geriatric patients and develop individualized, evidence-based treatment plans
• Deliver therapeutic interventions to improve strength, balance, mobility, and functional independence
• Document evaluations, treatments, and outcomes accurately in the EMR system
• Collaborate with an interdisciplinary team to coordinate comprehensive patient care
• Educate patients, families, and caregivers on home programs, safety, and functional strategies
• Participate in clinical education, mentorship, and ongoing professional development
• Contribute to a culture of respect, patient-centered care, and clinical excellence
