Physical Therapist

FOX Rehabilitation seeks a compassionate Physical Therapist to provide evidence-based geriatric rehab services in a patient-centered setting. You will evaluate and treat older adults, develop individualized care plans, and deliver functional, outcome-driven interventions to improve mobility, safety, and quality of life. Collaborate with an interdisciplinary team, document in EMR, and educate patients, families, and caregivers. Responsibilities

• Perform comprehensive physical therapy evaluations for geriatric patients
• Develop and implement individualized, evidence-based treatment plans
• Provide therapeutic exercises, gait training, balance and fall-prevention interventions
• Educate patients, families, and caregivers on home programs and safety
• Document evaluations, goals, and progress in EMR accurately and timely
• Collaborate with physicians, nurses, and other rehab professionals on patient care
• Monitor and adjust treatment plans based on patient progress and outcomes
• Participate in quality improvement and clinical excellence initiatives
• Support mentorship, training, and peer collaboration within the team
• Uphold FOX Rehabilitation's standards of professionalism, respect, and patient-centered care
