FOX Rehabilitation is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based geriatric rehabilitation, focusing on improving patient mobility, function, and overall quality of life across a variety of care settings. You'll collaborate within an interdisciplinary team to develop and implement individualized treatment plans, document progress, and educate patients and caregivers. Responsibilities

  • Perform comprehensive physical therapy evaluations for geriatric patients, including functional, strength, balance, and mobility assessments.
  • Develop and implement individualized, evidence-based plans of care focused on improving functional independence, safety, and quality of life.
  • Provide therapeutic interventions such as therapeutic exercise, balance and gait training, functional mobility training, and manual therapy as appropriate.
  • Educate patients, families, and caregivers on home exercise programs, fall prevention strategies, and techniques to support functional carryover.
  • Collaborate with an interdisciplinary care team, including physicians, nurses, and other rehabilitation professionals, to coordinate patient care.
  • Maintain accurate, timely, and compliant documentation in the electronic medical record, including progress notes, re-evaluations, and discharge summaries.
  • Monitor patient progress using objective outcome measures and adjust treatment plans based on clinical findings and patient goals.
  • Adhere to all professional, ethical, and regulatory standards for physical therapy practice within the state and organization.
  • Participate in professional development, mentorship, and continuing education opportunities offered by FOX Rehabilitation to support clinical growth and excellence.
  • Contribute to a culture of respect, teamwork, and high-quality patient-centered care within the practice.
