Nurse Practitioner or Physician Assistant

Conviva Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive primary care to adults 65+. In this role, you'll provide assessments, chronic disease management, preventive care, and same-day visits within a coordinated, value-based model. You'll collaborate with physicians, nurses, and care teams to develop personalized care plans, monitor outcomes, and support wellness programs that promote independence and quality of life. Responsibilities

• Provide comprehensive primary care to adults 65+, including assessments, diagnosis, and treatment.
• Manage chronic conditions through evidence-based care plans and regular follow-up.
• Deliver preventive services, wellness visits, and health screenings.
• Prescribe and manage medications in accordance with clinical guidelines and regulations.
• Review and interpret labs, imaging, and diagnostic results to guide care decisions.
• Document care accurately in the Electronic Medical Record (EMR) and track quality metrics.
• Collaborate with physicians, nurses, and care team members to coordinate value-based care.
• Educate patients and families on conditions, medications, and lifestyle modifications.
• Participate in quality improvement initiatives and population health efforts.
• Engage in ongoing professional development and contribute to a patient-first, team-based culture.
