Home Health RN Clinical Manager

CenterWell Home Health seeks a Home Health RN Clinical Manager to lead in-home clinical care for seniors and adults with chronic conditions. Responsibilities

• Lead and supervise home health clinical staff providing in-home care
• Oversee patient assessments and develop, review, and adjust care plans
• Ensure quality of care, regulatory compliance, and accurate documentation
• Coordinate interdisciplinary case conferences and collaborate with physicians
• Provide clinical guidance, coaching, and performance feedback to field clinicians
• Monitor clinical outcomes and implement quality improvement initiatives
• Support patient and family education on disease management and self-care
• Assist with scheduling, workload management, and resource allocation
