Home Health RN Clinical Manager

CenterWell Home Health seeks a Home Health RN Clinical Manager to lead in-home clinical operations for seniors and adults with chronic conditions. In this role, you'll oversee a team of nurses and clinicians, coordinate patient care plans, ensure regulatory and quality standards, and support safe, effective transitions of care. You'll manage caseloads, review documentation, coach and mentor staff, and collaborate with an interdisciplinary team to deliver personalized, patient-first home care that promotes independence and optimal outcomes.

Responsibilities

• Lead and supervise home health nursing and clinical staff
• Oversee development and coordination of individualized care plans
• Ensure compliance with home health regulations and quality standards
• Review and approve clinical documentation, including OASISManage patient caseloads and visit scheduling priorities
• Coach, mentor, and evaluate clinical team performance
• Collaborate with physicians and interdisciplinary team members
• Monitor clinical outcomes and implement quality improvement initiatives
• Support safe transitions of care and hospital readmission reduction
• Engage patients and families in education and care planning
