Home Health RN Clinical Manager

CenterWell Home Health seeks a Home Health RN Clinical Manager to lead in-home clinical operations for seniors and adults with chronic conditions. You will oversee a team of field clinicians, ensuring high-quality, patient-centered care, accurate OASIS and EMR documentation, and regulatory compliance. Responsibilities include care coordination, case conference leadership, staff mentoring, and performance improvement. Collaborate closely with an interdisciplinary team using innovative tools to support recovery, reduce hospitalizations, and help patients remain safely independent at home.

Responsibilities

• Lead and supervise home health clinical staff delivering in-home nursing care
• Oversee care coordination and interdisciplinary case conferences for assigned patients
• Ensure accurate, timely OASIS and EMR documentation and regulatory compliance
• Monitor clinical quality metrics and drive performance improvement initiatives
• Support safe discharge planning, rehospitalization prevention, and chronic disease management
• Provide coaching, mentoring, and performance feedback to field clinicians
• Collaborate with physicians, therapists, and caregivers to build individualized care plans
• Support scheduling and workload balance to meet patient needs and agency standards
