Charge Nurse

Touchstone Communities is seeking a Charge Nurse (LVN or RN) to lead clinical care in our skilled nursing and rehabilitation community. In this role, you'll coordinate and deliver resident-centered nursing care, supervise and mentor staff, and ensure regulatory and quality standards are met. You'll collaborate closely with therapy, physicians, and families to support recovery and long-term care goals. Responsibilities

• Supervise and coordinate nursing care for a designated unit or shift
• Provide direct patient care including assessments, medication administration, and treatments
• Lead and support LVNs, RNs, CNAs, and other care team members
• Ensure compliance with clinical protocols, regulatory standards, and documentation requirements
• Communicate with residents, families, and interdisciplinary team on care plans and progress
• Respond to changes in resident condition and initiate appropriate interventions
• Participate in care planning, admissions, discharges, and transitions of care
• Support orientation, coaching, and ongoing development of nursing staff
• Promote a safe, dignified, and person-centered care environment
• Collaborate with leadership to improve quality, satisfaction, and clinical outcomes
