Nurse Practitioner

Conviva Senior Primary Care is seeking a Nurse Practitioner to provide comprehensive primary care to adults 65+. In this role, you'll deliver preventive, chronic, and acute care, emphasizing wellness, education, and long-term patient relationships. You will collaborate closely with an interdisciplinary team to coordinate value-based, patient-centered care and support seniors' independence and quality of life. Responsibilities

• Conduct comprehensive geriatric assessments and routine primary care visits for adults 65+Diagnose and manage acute and chronic conditions using evidence-based guidelines
• Develop, implement, and adjust individualized care and wellness plans
• Prescribe medications and order/interpret diagnostic tests
• Coordinate care with physicians, nurses, social workers, and other team members
• Educate patients and families on disease management, prevention, and healthy lifestyles
• Monitor outcomes and participate in quality improvement and value-based care initiatives
• Maintain accurate, timely documentation in the electronic health record (EHR)
• Participate in care transitions, referrals, and community-based support programs
• Contribute to a compassionate, patient-first, and culturally sensitive care environment
