Registered Nurse Case Management

Ascension is seeking a Registered Nurse Case Management professional to coordinate patient care across our integrated healthcare system. In this role, you will perform comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to ensure safe, efficient transitions of care. You will educate patients and families, connect them with community resources, and advocate for quality, cost-effective outcomes in a compassionate, patient-focused environment that supports continuous learning and career growth.

Responsibilities

• Coordinate and manage patient care plans across the continuum of care
• Conduct comprehensive assessments and develop individualized case management plans
• Collaborate with physicians, nurses, and interdisciplinary teams to optimize outcomes
• Educate patients and families on care plans, resources, and community services
• Monitor patient progress, address barriers to care, and adjust plans as needed
• Ensure appropriate utilization of services and transitions of care
• Document assessments, care plans, and outcomes accurately in the EMRAdvocate for patient needs while supporting quality, safety, and compliance standards
