Responsibilities

  • Provide clinical leadership and oversight for home health services to ensure safe, high-quality patient care in the home setting
  • Supervise and support clinical teams, including nurses and therapists, promoting best practices and adherence to evidence-based guidelines
  • Collaborate with the Medical Director and leadership to develop and implement clinical policies, protocols, and quality initiatives
  • Review and oversee complex care plans, high-risk cases, and escalated clinical concerns to optimize patient outcomes
  • Monitor clinical performance metrics, patient outcomes, and compliance indicators, recommending and driving improvement actions as needed
  • Ensure compliance with federal, state, and accrediting body regulations, including CMS and applicable home health standards
  • Participate in case conferences, interdisciplinary team meetings, and care coordination activities to support holistic, patient-centered care
  • Provide clinical consultation and education to field clinicians on disease management, patient safety, and appropriate utilization of services
  • Support initiatives that enhance patient autonomy, functional status, and quality of life in the home environment
  • Contribute to strategic planning, program development, and innovation in in-home healthcare delivery
  • Foster a culture of compassion, integrity, and continuous improvement within the clinical team
