Signature HealthCARE is seeking a compassionate Registered Nurse to deliver high-quality, resident-centered care in our long-term and post-acute facility. The RN will assess, plan, implement, and evaluate nursing care; administer medications and treatments; coordinate with an interdisciplinary team; and provide education to residents and families. Responsibilities

  • Assess, plan, implement, and evaluate individualized nursing care for long-term and post-acute residents
  • Administer medications, treatments, and IV therapies safely and accurately
  • Monitor resident conditions, perform timely interventions, and communicate changes to providers and families
  • Document assessments, interventions, and outcomes in the EHR in accordance with regulatory standards
  • Collaborate with an interdisciplinary team to support resident-centered care plans and discharge planning
  • Educate residents and families on conditions, medications, and self-care strategies
  • Ensure compliance with infection control, safety, and quality standards
  • Support and mentor CNAs and other team members, modeling compassion, dignity, and servant leadership
  • Participate in quality improvement initiatives, audits, and care conferences
  • Contribute to a faith-based, mission-driven culture focused on respect, inclusion, and continuous improvement
