Registered Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Registered Nurse to provide high-quality clinical care to residents in our skilled nursing and rehab setting. The RN will complete assessments, administer medications and treatments, monitor changes in condition, and collaborate with the interdisciplinary team to develop and implement individualized care plans. The role includes supervising CNAs, educating residents and families, documenting accurately in the medical record, and supporting quality improvement initiatives. Responsibilities

• Provide direct nursing care to skilled nursing and rehab residents
• Conduct comprehensive assessments and monitor changes in resident condition
• Administer medications, treatments, and wound care per provider orders
• Develop, update, and implement individualized care plans with the care team
• Supervise, mentor, and support CNAs and other support staff
• Document care, assessments, and interventions accurately in the medical record
• Communicate with physicians, therapists, residents, and families regarding care
• Participate in care conferences and interdisciplinary team meetings
• Support infection control, safety, and regulatory compliance standards
• Contribute to quality improvement and clinical best-practice initiatives
