Registered Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Registered Nurse to provide high-quality skilled nursing care to short-term rehab and long-term care residents. The RN will complete assessments, administer medications and treatments, monitor resident conditions, and coordinate care plans with an interdisciplinary team. Responsibilities include supervising CNAs, documenting accurately in the medical record, educating residents and families, and supporting discharge planning. The ideal candidate is resident-centered, a strong team player, committed to clinical excellence, and comfortable working in a fast-paced, 24/7 care environment.

Responsibilities

• Provide skilled nursing care to rehab and long-term care residents
• Complete nursing assessments and update individualized care plans
• Administer medications, treatments, and wound care per provider orders
• Monitor and document resident status, responding promptly to changes
• Collaborate with physicians, therapists, and interdisciplinary team members
• Supervise and support CNAs and other direct care staff
• Educate residents and families on conditions, treatments, and discharge plans
• Maintain accurate, timely electronic medical records and documentation
• Follow infection control, safety, and regulatory standards in all care
• Participate in quality improvement and resident-centered initiatives
