Direct Support Program Coordinator

Sevita is hiring a Direct Support Program Coordinator to lead community-based services in our Healthcare & Medical Services, Community & Social Services programs. In this role, you will coordinate person-centered supports, oversee service plans, and supervise direct support staff to ensure safe, high-quality care. Responsibilities

• Coordinate and oversee direct support services for individuals in community-based programs
• Develop, implement, and monitor person-centered service plans
• Supervise, train, and coach direct support staff to ensure quality care
• Ensure compliance with healthcare, safety, and documentation standards
• Collaborate with families, clinicians, and community partners to support individual goals
• Monitor program outcomes and prepare required reports and documentation
• Participate in on-call support and respond to program issues as needed
• Support continuous improvement and incorporate best practices in services provided
