lead Direct Support Professional

Hope Network is seeking a Lead Direct Support Professional to guide a team providing person-centered care for adults with disabilities in community-based settings. In this role, you'll balance hands-on support with leadership responsibilities-coaching staff, coordinating schedules, and ensuring quality services. You'll assist individuals with daily living skills, health needs, and community integration, while implementing support plans and documenting progress. Responsibilities

• Provide hands-on support with daily living, personal care, and community integration for adults with disabilities.
• Lead, coach, and mentor direct support staff to ensure high-quality, person-centered services.
• Develop and implement individualized support plans in collaboration with interdisciplinary teams and families.
• Monitor client progress, document services accurately, and maintain required records and reports.
• Support behavior plans, implement crisis-prevention strategies, and ensure a safe, respectful environment.
• Coordinate schedules, assignments, and communication among team members to meet client needs.
• Administer or oversee medications per policy and maintain accurate health and incident documentation.
• Advocate for individuals' rights, preferences, and inclusion in community activities and employment.
• Participate in training, quality improvement, and team meetings to enhance service delivery.
• Promote Hope Network's culture of inclusivity, compassion, and professional growth.
