Traveler Changer

As an AMAP Traveler with Sevita in Healthcare & Medical Services, you'll deliver person-centered care across community and home settings, supporting individuals with diverse behavioral, medical, and developmental needs. You will assist with daily living, community integration, and medication administration per AMAP guidelines, while closely following individualized care plans. In this mobile role, you'll collaborate with families and care teams, ensure safety and accurate documentation, and travel between sites as scheduled. Responsibilities

• Provide person-centered in-home and community-based support to individuals with medical, behavioral, or developmental needs
• Assist with activities of daily living, transportation, and community integration
• Follow individualized care plans and accurately document services provided
• Monitor health, behavior, and safety, reporting changes to supervisors or clinical staff
• Collaborate with families, caregivers, and interdisciplinary teams to support goals
• Maintain professional boundaries and uphold confidentiality and ethical standards
• Travel between client homes and community settings as assigned
• Participate in training, coaching, and professional development opportunities
