As a Traveling MPA with Sevita, you'll provide community-based support across multiple locations, helping individuals access vital healthcare and social services. Responsibilities
- Provide mobile community-based support to individuals and families across assigned regions
- Conduct client assessments and assist with person-centered care and service plans
- Coordinate services with healthcare providers, social service agencies, and community resources
- Travel to homes and community locations to deliver on-site support and monitoring
- Document visits, progress notes, and service outcomes in compliance with regulations
- Educate individuals and families on available services, health, and safety resources
- Advocate for client needs and help navigate healthcare and social service systems
- Participate in team meetings, trainings, and case reviews to improve care quality
- Maintain confidentiality and adhere to all safety and compliance standards
- Contribute to a collaborative, inclusive culture focused on improving lives
