Traveling Operator

As a Traveling MPA with Sevita, you'll provide community-based support across multiple locations, helping individuals access vital healthcare and social services. You will conduct assessments, assist with person-centered care plans, and coordinate resources that promote safety, independence, and quality of life. Traveling regularly to homes and community settings, you'll document services, collaborate with providers, and advocate for client needs. Responsibilities

• Provide mobile community-based support to individuals and families across assigned regions
• Conduct client assessments and assist with person-centered care and service plans
• Coordinate services with healthcare providers, social service agencies, and community resources
• Travel to homes and community locations to deliver on-site support and monitoring
• Document visits, progress notes, and service outcomes in compliance with regulations
• Educate individuals and families on available services, health, and safety resources
• Advocate for client needs and help navigate healthcare and social service systems
• Participate in team meetings, trainings, and case reviews to improve care quality
• Maintain confidentiality and adhere to all safety and compliance standards
• Contribute to a collaborative, inclusive culture focused on improving lives
