Hospice Nurse

BJC HealthCare seeks a compassionate Hospice Nurse to provide expert end-of-life care in patients' homes and facilities across the St. Louis region. In this mission-driven role, you'll assess and manage symptoms, administer medications, coordinate interdisciplinary care, and support patients and families through complex physical, emotional, and spiritual needs. You'll collaborate with physicians, social workers, chaplains, and therapists, contribute to care planning and education, and document in the EMR while upholding BJC's standards of clinical excellence, respect, and cultural humility.

Responsibilities

• Provide direct hospice nursing care, including assessments, symptom management, and medication administration.
• Develop, implement, and update individualized hospice care plans with the interdisciplinary team.
• Educate patients and families on disease progression, comfort measures, and end-of-life options.
• Coordinate care with physicians, social workers, chaplains, and therapists to support holistic needs.
• Document assessments, interventions, and outcomes accurately in the EMR.
• Respond to changes in patient condition and adjust care plans and interventions accordingly.
• Support patients and families through emotional, cultural, and spiritual aspects of end-of-life care.
• Participate in case conferences, quality initiatives, and ongoing clinical education.
