Physical Therapist

Tender Touch Rehab Services seeks a compassionate Physical Therapist to provide evidence-based rehabilitation for seniors in skilled nursing and long-term care settings. You will perform comprehensive evaluations, design individualized treatment plans, and deliver hands-on interventions to maximize mobility, safety, and independence. Collaborating closely with an interdisciplinary care team, you'll support patient-centered goals, maintain accurate documentation, and educate patients, families, and caregivers. Responsibilities

• Provide individualized physical therapy evaluations and treatment plans for seniors in skilled nursing and long-term care settings
• Deliver evidence-based therapeutic interventions to improve mobility, function, and independence
• Collaborate with an interdisciplinary care team to coordinate patient-centered treatment goals
• Document assessments, progress notes, and outcomes in accordance with regulatory and company standards
• Educate patients, families, and caregivers on home exercises, safety, and functional strategies
• Participate in clinical case conferences, quality initiatives, and fall-prevention or restorative programs
• Supervise and mentor PTA staff as appropriate and support professional development
• Ensure compliance with Medicare and other payer regulations and facility policies
• Contribute to a respectful, compassionate environment that upholds patient dignity
• Engage in continuing education to maintain clinical excellence and current best practices
