Sevita is seeking a compassionate Therapeutic Counselor to join our Healthcare & Medical Services team. In this role, you will provide person-centered counseling, develop treatment plans, and deliver therapeutic interventions to individuals of all ages and abilities in home and community settings. You'll collaborate with an interdisciplinary team to support behavioral health needs, brain injury rehabilitation, and family systems. Responsibilities
- Provide individual, family, and/or group therapeutic counseling using evidence-based and trauma-informed approaches.
- Conduct biopsychosocial and behavioral assessments to identify strengths, needs, and treatment goals.
- Develop, implement, and regularly update individualized treatment and support plans in collaboration with individuals and families.
- Deliver community-based and home-based services, supporting skill-building, emotional regulation, and behavior management.
- Monitor client progress, adjust interventions as needed, and document all services accurately and promptly in the electronic health record.
- Coordinate care and communicate regularly with an interdisciplinary team, including internal and external providers.
- Provide crisis intervention, risk assessment, and safety planning, including coordination with higher levels of care when necessary.
- Educate individuals and families about behavioral health conditions, coping strategies, and available resources.
- Connect individuals and families to community resources and supports that promote stability and independence.
- Maintain professional boundaries, ethical standards, and compliance with all regulatory and organizational policies.
