Licensed Behavioral Health Counselor

VNS Health in New York City seeks a Licensed Behavioral Health Counselor to deliver compassionate home and community-based counseling services. You will conduct assessments, develop treatment plans, and provide individual and group therapy to adults and families. The role includes coordinating care with an interdisciplinary team, offering psychoeducation, and supporting crisis intervention. Responsibilities

• Provide individual and group counseling to clients with behavioral health needs in home and community settings
• Conduct biopsychosocial assessments and develop person-centered treatment plans
• Coordinate care with physicians, nurses, social workers, and community resources
• Monitor client progress, adjust interventions, and maintain accurate clinical documentation
• Educate patients and families on coping strategies, resources, and treatment options
• Ensure compliance with regulatory, ethical, and documentation standards
• Participate in case conferences, team meetings, and interdisciplinary care planning
• Support crisis intervention and risk assessment as needed
