Licensed Social Worker, Hospice Home Care

VNS Health is seeking a Licensed Social Worker for Hospice Home Care to support patients and families across New York City. In this role, you will complete psychosocial assessments, develop person-centered care plans, and provide counseling, crisis intervention, and end-of-life support in the home. You will collaborate closely with nurses, physicians, chaplains, and caregivers to coordinate community resources and benefits. The position emphasizes culturally sensitive care, accurate documentation, and advocacy to help patients maintain dignity, comfort, and emotional well-being throughout the hospice journey.

Responsibilities

• Conduct comprehensive psychosocial assessments for hospice patients and families in the home
• Develop and update individualized care plans aligned with patient goals and hospice standards
• Provide counseling, grief support, and crisis intervention to patients and caregivers
• Educate families on hospice philosophy, available services, and community resources
• Coordinate services with nurses, physicians, chaplains, and other team members
• Advocate for patient rights, advance care planning, and access to benefits
• Document all assessments, interventions, and outcomes in the electronic health record
• Participate in interdisciplinary team meetings and case conferences
• Facilitate family meetings to support decision-making and conflict resolution
• Maintain compliance with regulatory, ethical, and agency standards of practice
