CASE MANAGER

Encompass Health Mechanicsburg is seeking a dedicated Case Manager to join our collaborative team focused on providing top-tier care in the realm of healthcare and medical services. In this role, you will manage a diverse patient caseload, ensuring seamless treatment coordination, comprehensive patient support, and optimal use of resources. This position requires a strong ability to work collaboratively with medical professionals to develop patient-specific plans, striving for enhanced recovery and efficient patient throughput.

Responsibilities

• Assess new patients, reviewing their medical condition, needs, and personal preferences.
• Develop, evaluate, and update individualized patient care plans in conjunction with healthcare providers.
• Coordinate necessary services and care for patients, from admission through to aftercare.
• Act as a liaison between patients, families, healthcare teams, and additional resources.
• Regularly review case progress, re-evaluating patient plans as necessary.
• Maintain comprehensive documentation of patient condition, care, and progress.
• Educate patients and families about health conditions and treatment plans.
• Advocate for patient needs and optimal care delivery within the healthcare system.
