HOME OF HOPE seeks an HTS/Direct Support Professional to provide compassionate, hands-on support to individuals in a faith-based residential setting. In this role, you will assist residents with daily living activities, personal care, and medication reminders while promoting independence and dignity. You'll implement individualized service plans, support life-skills training, and document progress and incidents accurately. Working closely with a multidisciplinary team, you'll help maintain a safe, structured, and nurturing environment that reflects our mission to break cycles of homelessness and foster long-term stability for children and families in crisis.

Responsibilities

  • Provide direct daily living support and supervision to residents in a group home setting
  • Implement individualized service plans, goals, and behavioral support strategies
  • Assist residents with personal care, hygiene, meal prep, and medication reminders per policy
  • Support life skills training, education, and recreational activities for residents
  • Maintain accurate documentation, progress notes, and incident reports in a timely manner
  • Ensure a safe, clean, and nurturing environment aligned with agency policies and faith based values
  • Collaborate with supervisors, case managers, and clinicians to coordinate care
  • Respond calmly to crises and follow de escalation and emergency procedures
  • Communicate professionally with families, guardians, and external partners as needed
  • Participate in training, staff meetings, and ongoing professional development
