Direct Care For Supported Living

Tomorrow's Hope Foundation is seeking compassionate and dedicated individuals for the position of Direct Care Support in our Supported Living services, dedicated to assisting individuals with disabilities in achieving a more independent and fulfilling life. In alignment with our commitment to uphold the dignity and quality of life of those we serve, this role involves personal care, support in daily activities, and engagement in educational and social activities. Responsibilities

• Assist with personal care tasks including but not limited to: feeding, grooming, and bathing.
• Implement and monitor individual support plans designed to meet residents' unique needs and goals.
• Encourage and assist residents in engaging in community activities and educational programs.
• Monitor health and wellbeing of residents, and report any changes in condition to the appropriate professional.
• Facilitate communication between residents, family members, and medical professionals as required.
• Maintain accurate records of residents' progress, routines, and daily activities.

Required Skills

• Experience in managing and implementing care plans
• Ability to perform physical tasks involved with care duties
• Strong communication and interpersonal skills
• Adaptable to various situations with a compassionate demeanor
• Basic medical knowledge, including first aid
