Responsibilities
- Provide strategic leadership for the Social Work department, aligning services with organizational goals and regulatory standards.
- Oversee, mentor, and develop social work staff, fostering a culture of collaboration, accountability, and continuous learning.
- Design, implement, and evaluate patient-centered psychosocial programs that improve care coordination and health outcomes.
- Ensure compliance with federal, state, and accreditation requirements for social work and case management services.
- Collaborate with medical, nursing, and rehabilitation leaders to integrate social work into interdisciplinary care plans.
- Manage departmental budgets, staffing models, and performance metrics to optimize quality and efficiency.
- Lead quality improvement initiatives, using data to drive enhancements in patient satisfaction and care transitions.
- Develop and maintain relationships with community agencies, payers, and external partners to support patient needs.
- Create and maintain policies, procedures, and documentation standards for social work practice.
- Champion diversity, equity, and inclusion within the department, supporting culturally competent, trauma-informed care.
