Choice Lead Case Manager

The Choice Lead Case Manager at the Housing Authority of Kansas City, MO leads case management services for Housing Choice participants, coordinating housing stability, supportive services, and goal planning. Responsibilities

• Lead and coordinate case management services for Housing Choice participants to support housing stability and self-sufficiency.
• Supervise and mentor case managers, monitor caseloads, and provide guidance on complex resident situations.
• Conduct or oversee client assessments, develop individualized service plans, and track resident progress toward goals.
• Ensure program compliance with HUD, HAKC, and other regulatory requirements, including documentation and reporting.
• Maintain accurate case records, data entry, and outcome tracking in case management systems.
• Build and maintain partnerships with community agencies to connect residents with employment, education, and support services.
• Respond to resident concerns, mediate conflicts, and escalate issues appropriately to protect housing stability.
• Support continuous improvement of case management processes, policies, and resident service strategies.
