Registered Nurse

Keene Center, a Genesis HealthCare skilled nursing and rehabilitation facility in Keene, NH, is seeking a compassionate Registered Nurse to deliver resident-centered care for short-term rehab, long-term care, and memory support residents. The RN provides clinical assessments, medication administration, wound care, and care coordination while promoting dignity, safety, and quality of life. Responsibilities

• Provide direct nursing care to short-term rehab and long-term care residents in accordance with care plans and physician orders
• Administer medications and treatments, monitor resident condition, and document assessments accurately in the medical record
• Coordinate with interdisciplinary team members to develop, update, and implement individualized care plans
• Educate residents and families on care needs, medications, and health management, supporting informed decision-making
• Supervise and support CNAs and other support staff to ensure safe, high-quality resident-centered care
• Respond promptly to changes in resident status and emergencies, following facility and regulatory protocols
• Ensure compliance with infection control, safety, and regulatory standards in all aspects of care
• Participate in quality improvement initiatives, audits, and training to enhance clinical outcomes
• Communicate effectively with physicians, therapists, families, and team members to support continuity of care
• Document all care, incidents, and communications in accordance with facility policies and legal requirements
