Direct Support Professional

As a Direct Support Professional at Sevita, you'll provide person-centered care to individuals with disabilities in home and community settings. You'll support daily living needs, including personal care, meal preparation, transportation, and community outings while promoting independence and dignity. You'll follow individualized support plans, document progress, and communicate with families and team members. Responsibilities

• Provide direct care and support to individuals with disabilities in home and community settings
• Assist with daily living activities, personal care, meals, and transportation
• Implement person-centered support plans and behavior strategies
• Monitor, document, and report changes in condition and progress
• Promote independence, dignity, and community participation
• Follow health, safety, and infection control procedures
• Communicate effectively with individuals, families, and team members
• Participate in ongoing training and professional development
