CommuniCare is seeking a Supported Employment Specialist to help individuals with disabilities or barriers to work find and keep meaningful jobs. In this resident-centered role, you will assess strengths and interests, develop individualized employment plans, provide job readiness coaching, and coordinate vocational supports with clinical and care teams. You will build relationships with local employers, support job placement and on-the-job training, and monitor progress to promote long-term success, independence, and community integration in a compassionate, team-focused environment.

Responsibilities

  • Conduct vocational assessments to identify strengths, needs, and employment goals
  • Develop and update individualized employment plans in collaboration with residents and care teams
  • Provide job readiness training, including resume building, applications, and interview practice
  • Coordinate and accompany residents to employer meetings, interviews, and job sites as needed
  • Deliver on-the-job coaching and support to ensure successful job placement and retention
  • Collaborate with clinical, rehabilitation, and social work staff to integrate employment goals into care plans
  • Maintain accurate documentation, progress notes, and required reports in a timely manner
  • Educate employers on disability awareness and reasonable accommodations to support inclusive hiring
  • Monitor employment outcomes and adjust supports to promote long-term success and independence
  • Participate in team meetings, trainings, and quality improvement initiatives to enhance services
