Outreach Specialist

Element Care seeks an Outreach Specialist for the North Shore to connect frail seniors and caregivers with our PACE program. In this role, you will build relationships with community partners, conduct home and community visits, and educate families about services that help seniors remain safely at home. You'll identify and qualify prospective participants, support the enrollment process, and track outreach activities. Working in a compassionate, interdisciplinary team, you'll promote Element Care's mission while growing your skills in community-based healthcare and senior services.

Responsibilities

• Conduct community and home visits to educate seniors and caregivers about Element Care's PACE program
• Develop and maintain relationships with referral sources, community agencies, and healthcare providers
• Identify, screen, and qualify prospective participants for PACE eligibility
• Support seniors and families through the enrollment process, coordinating closely with clinical and administrative teams
• Plan and participate in outreach events, presentations, and information sessions across the North Shore
• Maintain accurate records of outreach activities, referrals, and outcomes in designated systems
• Collaborate with interdisciplinary staff to align outreach efforts with participant needs and organizational goals
• Represent Element Care's mission and values with compassion, professionalism, and cultural sensitivity
