Life Skills Coach

Heartwell Services, LLC seeks a Life Skills Coach to provide in home support to seniors and adults with disabilities. In this role, you will coach clients in daily living skills, support safe routines, and offer dependable companionship in their homes. Responsibilities include assisting with personal care, meal preparation, light housekeeping, and community access while following person centered care plans. You'll document visits clearly, report concerns promptly, and collaborate with a small, mission driven team to help clients live safely, independently, and with dignity at home.

Responsibilities

• Coach clients in daily living skills (personal care, routines, community access).
• Provide companionship and encourage engagement in meaningful activities.
• Support safe mobility and a secure in home environment.
• Assist with meal preparation, light housekeeping, and basic home tasks.
• Follow person centered care plans and document visits accurately.
• Communicate changes in client needs to supervisors promptly.
• Build trusting, respectful relationships with clients and families.
• Collaborate with a small care team to ensure consistent, reliable support.
