Element Care, a nonprofit PACE organization, seeks an Outreach Specialist to build awareness of our all-inclusive care program for frail seniors across the North Shore. In this field-based role, you will cultivate relationships with hospitals, clinics, senior housing, and community agencies, educate families and professionals about PACE, and generate qualified referrals. You'll conduct presentations, attend events, and support enrollment activities while tracking outreach metrics. Join a compassionate, interdisciplinary team helping seniors remain independent in their communities.

Responsibilities

  • Develop and execute outreach plans to promote Element Care's PACE services across the North Shore.
  • Build and maintain relationships with hospitals, clinics, senior housing, and community organizations.
  • Educate seniors, families, and professionals about PACE eligibility, benefits, and enrollment processes.
  • Conduct community presentations, health fairs, and outreach events to generate referrals.
  • Support prospective participants and caregivers through the referral and pre-enrollment process.
  • Collaborate with interdisciplinary care teams to ensure smooth handoff from outreach to enrollment and care.
  • Track outreach activities, leads, and conversions using CRM or tracking tools and report on performance.
  • Represent Element Care's mission and values in the community with professionalism and cultural sensitivity.
  • Provide feedback from the field to inform outreach strategies and marketing materials.
  • Travel regularly within the North Shore region to community sites and events.
