Core Clinical Partners is excited to support its partnership with Portneuf Medical Center and Portneuf Medical Group and is seeking Nurse Practitioners and Physician Assistants to join a well-established Hospital Medicine program in Pocatello, Idaho. Through this partnership, APPs are employed by Portneuf Medical Group while Core Clinical Partners provides leadership, operational support, recruitment, and practice management services.

Position Overview

Join a collaborative Hospital Medicine team at Portneuf Medical Center, a 205-bed regional referral center serving Eastern Idaho and surrounding communities. This established program manages an average daily census in the 90s and features dedicated day, swing, and nocturnist coverage supported by experienced physicians, strong nursing partnerships, and broad specialty support.

APPs play an integral role in the care team, managing hospitalized patients in collaboration with Hospital Medicine physicians while caring for a diverse and medically complex patient population. With a closed ICU model, intensivist support, no procedures required, and Epic EMR, APPs can focus on delivering exceptional patient care in a collaborative and well-supported practice environment.

Responsibilities

  • Evaluate and manage hospitalized adult patients in collaboration with supervising physicians
  • Perform admissions, rounding, discharges, and patient assessments
  • Coordinate patient care with physicians, specialists, nursing, case management, and ancillary services
  • Monitor changes in patient condition and assist with treatment planning
  • Participate in interdisciplinary rounds and patient handoffs
  • Support quality initiatives, patient throughput, and patient satisfaction efforts
  • Accurately document patient encounters within Epic EMR
  • Provide high-quality, patient-centered inpatient care

Qualifications

  • Acute Care Nurse Practitioner (ACNP) or Physician Assistant preferred
  • Family Nurse Practitioners with significant Hospital Medicine, Emergency Medicine, or Critical Care experience will be considered
  • Eligible for Idaho licensure
  • Hospital Medicine experience preferred
  • Strong communication and teamwork skills
  • Commitment to high-quality, patient-centered care
